Community Health Coordinator (NTHC) PRN

  • Work hours: PRN – As need; 3 or 4 days/week; 20 hours/week; some Saturday's as needed depending on the business need.
  • Salary range: $18.64 – $27.82 per hour (based on relevant experience)

  • High School Diploma or Equivalent required And
  • Bachelor's Degree preferred.


  • 1 year Community health experience working with culturally diverse individuals providing program coordination support to include: at least 6 months Demonstrated experience coordinating volunteer services/programs serving diverse populations, managing data collection and reporting, organizing and synthesizing information from multiple sources and partners, and managing relationships with external organizations.
  • 6 months experience in Food Systems efforts or working to address food insecurity preferred.


  • Bilingual Preferred: English and Spanish
  • Strong verbal and written communication skills and the ability to maintain composure in stressful situations imperative.
  • Ability to problem solve, time management skills, detail-oriented, ability to work independently as well as with others, strong organizational & prioritization skills, and ability to motivate and inspire others.
  • Entry level experience using Excel required; intermediate level experience preferred.

Position Responsibilities:


The North Texas Health Communities (NTHC) Community Health Coordinator will coordinate Food Access programs for Blue Zones Project and North Texas Healthy Communities. This includes program operations support and data collection efforts for any of the active Food Access Policy programs operating within the department, like the Double Up Food Bucks program.

Essential Functions:

  • Assist in program coordination of various Food Access Policy programs which includes organizing efforts to ensure applications, MOUs and other important program documents are up to date and saved in the appropriate shared folder.
  • Support data collection efforts by regularly requesting data from partners, uploading documents to department shared drive, and performing data entry as needed.
  • Develop and effectively manage program timelines to ensure timely completion of program deliverables.
  • Disseminate programmatic and marketing materials when required, coordinate community surveys, and convene focus groups on a determined cadence.
  • Develop presentations and other materials for use during program-related events, including assisting with training of store managers, employers, and/or anyone at the store/market level when on-boarding a new grocery store and/or Farmer's Market.
  • Serve as programmatic liaison to internal and external stakeholders as needed under the supervision of the Policy leader.
