Sr. Practice Transformation Specialist.

Texas Health Resources is seeking a Sr. Practice Transformation Specialist. The position will be based out of North Point in Dallas, TX. Full Time: 1st Shift 40 hoursMonday-Friday LOCATION: Texas Health Organization for Physicians North Point I 9229 LBJ Frwy. Dallas, TX 75423 SALARY: Commensurate with experience $29.51 - $48.28 per hour Position Summary:This position requires specific expertise and knowledge to assist providers and practices achieve national, regional and local recognition. This position will require staff to be uniquely qualified and trained in available recognitions.

 

Job Responsibilities:• Provide education and coaching on recognitions requirements: Mentor, support, and provide continuous education to physicians, practice staff, and Practice Transformation Specialist on the Patient Centered Medical Home (PCMH) principles. Lead learning collaborative sessions. Manage and track timelines for learning collaboratives; assign tasks and responsibilities to Practice Transformation Specialist. Develop and review learning collaborative presentations and resources. Assess practices in the adoption on the medical home model. Facilitate practice assessments and conduct gaps analysis for a baseline understanding of opportunities of transformation. Facilitate and teach the utilization of appropriate tools and methodologies, such as Define/Measure/Analyze/Improve/Control (DMAIC), Lean/Six Sigma, and/or rapid Plan/Do/Study/Act (PDSA) cycles for improvement. Review supporting documentation for practices who are submitting for recognitions and provide feedback and support where documentation does not align with the concepts • Deploy and manage quality improvement initiatives: Develop and plan operational interventions and campaigns. Make adjustments to interventions as needed. Manage project timelines and milestones using appropriate project management tools; track milestones and deliverables. Assign task and responsibilities to Practice Transformation Specialist. Lead the development, implementation, and monitoring of multiple performance improvement activities network wide as well as at a practice. Report status updates to Clinical Outcomes, Manager • Maintain knowledge of regulatory requirements: Review and understand regulatory requirements analyze. Analyze and communicate to practice staff, leadership, and junior level employees changes that may impact departmental procedure, policies, and processes. • Other duties assigned

 

Qualifications

Required Minimum Education:• Bachelor's Degree in healthcare, social services, business, or public health related areaRequired Minimum Experience:• 6 years' experience in health care delivery to include 3 years in quality improvement or performance improvement as a primary role, preferably in a primary care setting• 4 years' experience leading national recognition programRequired Certifications:• SSGBC - Six Sigma Green Belt Certification upon hire• NCQA PCMH CCE Certified upon hireSkills and Abilities:• Advance knowledge in Microsoft office suite 2013• Strong attention to detail• Strong collaboration skills – ability to achieve results through partnerships• Ability to utilize sound decision making and problem-solving skills; and think critically to act in a sense of urgency• Knowledge of quality programs and regulatory requirements such as MACRA, MIPS, CMS, NCQA, and other payor contracts• knowledge of performance improvement processes, synergy principles and change management facilitation• Strong project management skills; ability and experience with managing multiple projects simultaneously and the ability to monitor and document project timelines, activities, risks and issues• Ability to produce high quality deliverables in a fast paced; deadline sensitive environment
